Home-Based Support Services

The goal of the Home-Based Support Services Program is to provide assistance to individuals with disabilities so that they can remain independent in their own homes or living with family. The support services are tailored to meet the evolving needs of individuals, as well as their personal preferences. The HBS program is funded through the Adult Medicare Waiver or the Children’s Support Waiver.

Individuals and their guardians may choose from an array of separate HBS services, up to a monthly dollar cost maximum set by DHS, which best meets the changing needs of the individual. All services provided must be for the direct benefit of the individual and must be directly related to their disability.

To register, you must make an appointment with Great Rivers Service Coordination Unit (217-243-2330) to determine your eligibility.
